Medicaid can be important to get you or a loved one the needed care to remain at home without worrying about the cost of care. Understanding of the Medicaid process and issues involved is an invaluable benefit to those looking for assistance in accessing long-term, affordable home or institutional care.
This presentation will be an introduction to Medicaid and what you need to know.
Topics to be discussed include, but not limited to:
Community Medicaid (Home Care).
- list of the document that are required when applying for Medicaid
- FAQ about Pooled Income Trusts.
- Community Medicaid Eligibility:
- Pooled Income Trust for surplus income (see attached FAQ).
- Documents Needed (any questions about the documents on the list)
- Medicaid Application and submission
- Requesting home care after approval.
- Also "touch on" Institutional Medicaid (nursing home) and answer any questions people have .

Susan Marchesini, Medicaid Coordinator
Susan Marchesini has been a Medicaid Coordinator for over 20 years, working with elder law firms preparing Institutional (nursing home) and Community (home care) Medicaid applications for their clients. She assists with client’s preparation for Medicaid eligibility, gathering the necessary documents, preparing and filing the Medicaid application and assisting with the implementation of home care services. Susan also acts as liaison between the client and the Human Resource Administration or Department of Social Services and as the initial liaison between the Pooled Income Trust and client to get the process up and running when a client is applying for Medicaid home care services.
